Online registration to become participants of the Camp Lejeune Contaminated Water Disease Database
Database survey start date: March 22, 2011
The deadline for submission of all information and survey data will be February 29, 2012.
We will continue to receive submissions after the deadline date for future comparative database reports.
Database results will be posted on this site when the processing has been completed.

Directions when applying your information.
1. Begin by viewing the diseases by clicking on the arrow to the right of the page in the drop down box.
2. After viewing the diseases click on the disease you wish to submit, it will turn Blue.
3. Each disease should be typed into the box under “Enter Disease below from the Browse by Disease drop down box above ”.
4. Yes you can add more than one disease, if you have the disease type it in. After you have typed a disease you can add other diseases by repeating steps 1 - 3.
5. In the sections "Please check appropriate box" Please check the box which applies to you.
6. Please fill in the sections "Enter Dates" and "Email address" then click the "Submit Button"
